Our patient portal, myEMHShealth, is a secure way for you to access your health information anytime, anywhere. With myEMHShealth, you can:
  • Manage your appointments      
  • Request prescription refills  
  • Send secure messages to your healthcare team
  • View and print details of your health record, such as: Lab results, medications, allergies, immunizations, etc.
 


To enroll at your practice you will need:

  • A photo ID
  • Email address
After letting your practice know you'd like access to myEMHShealth, you will be sent an email inviting you to join our patient portal.
 


To self-enroll, you will need to enter:

  • Your first name and last name
  • Date of birth
  • Medical Record Number (MRN) - This can be found on the visit summary you received at the of your last office visit, ER visit, or inpatient stay
