Confidentiality

Patients 18 years of age or older are responsible for their own personal health record. As a parent, you no longer have the ability to request medical information concerning your son or daughter without their consent. However, your son or daughter may release specific information to you by filling out a release form that they can obtain from the Cutler Health Center.

If a patient of the Cutler Health Center needs to obtain their medical record, they may do so by calling (207) 581-4000.
